Применение 2-хлордезоксиаденозина в терапии гистиоцитоза из клеток Лангерганса у детей

   Стандартная терапия пациентов высокого риска с гистиоцитозом из клеток Лангерганса (ГКЛ) ассоциирована с существенным риском неудачи терапии. В настоящем исследовании проанализированы результаты применения 2-хлордезоксиаденозина в качестве альтернативной терапии у пациентов данной группы. В исследование включено 30 пациентов с мультисистемной формой ГКЛ с вовлечением органов риска. Девятнадцать пациентов получили в 1-й линии терапию с использованием преднизолона и винбластина (стандартная группа). Девять пациентов получили терапию 2-СdA и цитозин-арабинозидом (группа 2-CdA). Пациенты, получившие стандартную терапию, достоверно чаще (55,5 % против 0 %, р = 0,0095) нуждались в проведении терапии 2-й линии. Перманентные последствия заболевания чаще развивались в группе стандартной терапии (63,6 % против 0 %, р = 0,0147). Общая выживаемость среди пациентов стандартной группы составила 65,7 ± 12,8 % и в группе 2-CdA – 88,8 ± 10 %, p = 0,27. Бессобытийная выживаемость в стандартной группе составила 33,9 ± 12 %, в группе 2-CdA – 88,8 ± 10 %, p = 0,0207. Применение 2-CdA сопровождалось высокой гематологической токсичностью. Терапия с использованием 2-CdA высокоэффективна в лечении пациентов высокого риска с гистиоцитозом из клеток Лангерганса. Применение препарата должно быть ограничено специализированным стационаром.
