СТАНОВЛЕНИЕ ДОНОРСКОГО ХИМЕРИЗМА У ПАЦИЕНТОВС ПЕРВИЧНЫМИ ИММУНОДЕФИЦИТАМИ ПОСЛЕ АЛЛОГЕННОЙ ТРАНСПЛАНТАЦИИ ГЕМОПОЭТИЧЕСКИХ СТВОЛОВЫХ КЛЕТОК

Аннотация

Аллогенная трансплантация гемопоэтических стволовых клеток (аллоТГСК) применяется для лечения широкого круга первичных иммунодефицитов (ПИД). Исследование особенностей становления гемопоэтического химеризма у пациентов с различными ПИД и его связи с исходом аллоТГСК представляет большой интерес. В исследование включены 16 аллоТГСК у пациентов с ПИД. Трехлетняя общая выживаемость составила 72,2 ± 12,0 %. У 13 (81,25 %) пациентов удалось достигнуть полного донорского химеризма (ПДХ). У 3 (18,75 %) пациентов наблюдалось длительное персистирование смешанного химеризма (СХ). При СХ в периферической крови Т-клеточная субпопуляция представлена в основном клетками донора, а гранулоциты преимущественно или полностью клетками реципиента; уровень химеризма в субпопуляции В-лимфоцитов значительно различается от их полного отсутствия (0 % химеризм) до ПДХ. У пациентов с ПИД может наблюдаться приживление отдельных клеточных линий (расщепленный химеризм). У некоторых пациентов произошло значительное снижение химеризма в течение 1 года с последующей стабилизацией. Увеличивающийся СХ не ассоциирован с отторжением трансплантата при ПИД. Развитие ПДХ у пациентов с ПИД обеспечивает восстановление всех клеточных линий, участвующих в иммунном ответе независимо от диагноза, однако сопряжено с более частым развитием РТПХ, которая является серьезным осложнением аллоТГСК и может стать причиной cмертности, связанной с лечением (treatment-related mortality, TRM). СХ/расщепленный химеризм, при котором меньше частота развития реакции «трансплантат против хозяина» (РТПХ), также может обеспечить формирование полноценного иммунного ответа и коррекцию других проявлений заболевания, но только при замещении дефектных линий клеток в зависимости от диагноза.
