Показатели качества жизни, профиль симптомов и клинический эффект терапии второй линии дазатинибом у больных хроническим миелолейкозом при резистентности или непереносимости предшествующей терапии иматинибом: результаты 2-летнего мониторинга

В статье представлены результаты многоцентровой наблюдательной программы «Качество жизни и профиль симптомов у больных хроническим миелолейкозом в хронической фазе заболевания в отдаленные сроки 2-й линии терапии» (2012– 2015 гг.). В условиях реальной клинической практики проведен анализ эффективности и безопасности терапии 2-й линии дазатинибом у 30 больных в хронической фазе хронического миелолейкоза (ХМЛ) в течение 2 лет после начала лечения на основании клинических данных и оценок препарата пациентами, включая оценку показателей качества жизни и про- филя симптомов.

Средний возраст больных составил 48 лет (стандартное отклонение – 13,1 года), соотношение мужчины / женщины 14 / 16, индекс коморбидности – от 0 до 5 баллов. Все больные в качестве 2-й линии терапии ХМЛ получали дазатиниб в дозе 100 мг / сут. Обследование больных проводили через 12, 18 и 24 мес после начала 2-й линии терапии. Полный гематологический ответ (ГО) достигнут или сохранен у 96,3 % пациентов, полный цитогенетический ответ – у 66,6 %, полный, или большой, молекулярный ответ – у 60 % больных. Применение дазатиниба в течение 2 лет терапии сопровождалось хорошей переносимостью: гематологические и негематологические нежелательные явления III–IV степени наблюдались редко и ни в одном случае не привели к отмене препарата. Оценку качества жизни проводили с помощью общего опросника оценки качества жизни – RAND SF-36, оценку спектра симптомов – с помощью опросника оценки симптомов у больных ХМЛ (Comprehensive Symptom Profile-Leuk-CML, CSP Leuk-CML). Статистическую значимость различий показателей оценивали с помощью метода «обобщенные уравнения оценки» (generalized estimated equation, GEE). Показатели физического, ролевого физического и ролевого эмоционального функционирования, жизнеспособности и психического здоровья у больных ХМЛ значительно улучшились по сравнению с соответствующими показателями до начала 2-й линии терапии (р < 0,05); выраженность некоторых актуальных симптомов стала существенно ниже, чем до начала 2-й линии терапии; уменьшилась доля пациентов, имеющих значительно выраженные симптомы (р = 0,01). Через 24 мес после начала терапии дазатинибом у 83 % больных ХМЛ зарегистрирован ответ на лечение относительно качества жизни в виде улучшения или стабилизации состояния.

Результаты наблюдения ограниченного числа больных ХМЛ в условиях реальной клинической практики подтвердили данные о клинической эффективности и безопасности терапии дазатинибом, полученные в клинических исследованиях, а также продемонстрировали важность учета мнения больного при оценке эффекта и безопасности терапии дазатинибом в отдаленные сроки.
