Research results indicate the presence of antigenic effects leading to pathological activation and dysregulation of the T cells in the bone marrow with increased production of pro-inflammatory cytokines that damage hematopoietic stem cells. The triggering factor that initiates the cascade of immune reactions is currently unknown. The high efficiency of immunosuppressive therapy (IST), which allows achieving remission in the majority of AA patients, is evidence of the immune genesis of the disease. The pathogenesis of AA is currently being actively studied. The participation of T cell subpopulations in immune response is beyond doubt, but the issues of their significance in the AA pathogenesis have not been fully studied. A more detailed understanding of the AA development mechanisms is necessary for the development of long-term effective treatment.</p><p><bold>Aim. </bold>To investigate the T cells subpopulation in bone marrow of AA patients during IST.</p><p><bold>Materials and methods. </bold>The study included 41 patients over 18 years of age with newly diagnosed acquired AA without previous IST. Treatment was carried out according to a protocol including horse antithymocyte globulin and cyclosporine. Flow cytometry was used to detect T cell subpopulations. Bone marrow examination was performed at three time points: initially, 3 and 6 months after initiation of combined IST.</p><p><bold>Results. </bold>Multidirectional changes in the T cell subpopulations ratio before the start of IST were found in all AA patients included in the prospective study: most patients had a higher proportion of effector CD4<sup>+</sup> and CD8<sup>+</sup> T cells (61 and 83 % of patients, respectively), memory CD4<sup>+</sup> T cells (63 % of patients), and a lower proportion of naive CD4<sup>+</sup> and CD8<sup>+</sup> T cells (81 and 51 % of patients, respectively) compared to donors (<italic>p </italic>&lt;0.05). The abnormal T cell subpopulation ratios that cause abnormal immune response are most pronounced in the very severe form of AA. When responding to treatment, no significant changes in T cells subpopulation were found either before the start of IST or 6 months after compared with donors. In the absence of treatment response, a higher proportion of effector CD4<sup>+</sup> and CD8<sup>+</sup> T cells, memory CD4<sup>+</sup> T cells, and a lower proportion of naive CD4<sup>+</sup> and CD8<sup>+</sup> T cells were detected compared to donors already before the start of IST (<italic>p </italic>&lt;0.05). Of all cytometric parameters, a significant relationship was obtained between effector CD8<sup>+</sup> T cells dynamics and response to treatment (<italic>p </italic>= 0.040). The number of these cells at the onset and control points significantly correlated with the response to IST. In patients who have not responded to treatment by the 3rd month from IST initiated, there is a further increase in effector CD8<sup>+</sup> T cells number, despite the IST continuation, which dictates the need to intensify IST in the shortest possible time (between the 3rd and 6th months) in these patients, namely, a 2nd course of equine antithymocyte globulin and continuation of cyclosporine therapy or individual consideration of alternative treatment (allogeneic hematopoietic stem cell transplantation).</p><p><bold>Conclusion. </bold>Studying the T cells subpopulation dynamics, in particular effector T cells, allows us to determine the tactics of further treatment in the early stages of IST in order to select the optimal treatment program for each AA patient.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение. </bold>Апластическая анемия (АА) – неопухолевое и редко встречающееся заболевание системы крови, характеризующееся глубокой панцитопенией вследствие развития аплазии костного мозга при иммуноопосредованном повреждении пула гемопоэтических стволовых клеток. Результаты исследований указывают на наличие антигенного воздействия, приводящего к патологической активации и дисрегуляции Т-клеточного звена в костном мозге с повышенной продукцией провоспалительных цитокинов, повреждающих гемопоэтические стволовые клетки. Пусковой фактор, запускающий каскад иммунных реакций, на сегодняшний день не известен. Высокая эффективность иммуносупрессивной терапии (ИСТ), позволяющей достичь ремиссии у большинства больных АА, является свидетельством иммунного генеза заболевания. Патогенез АА в настоящее время активно изучается. Участие субпопуляций Т-клеток в процессах иммунного ответа не вызывает сомнений, однако вопросы их значения в патогенезе АА до конца не изучены. Более детальное понимание механизмов развития АА необходимо при разработке долгосрочного эффективного лечения.</p><p><bold>Цель исследования </bold>– изучить субпопуляционный состав Т-клеток больных АА в костном мозге в процессе ИСТ.</p><p><bold>Материалы и методы. </bold>В исследование был включен 41 больной старше 18 лет с впервые приобретенной АА без предшествующей ИСТ. Лечение проводилось по протоколу, включающему лошадиный антитимоцитарный глобулин и циклоспорин. Для детекции субпопуляций Т-клеток использовался метод проточной цитометрии. Исследование костного мозга проводилось в 3 точках: дебют, через 3 и 6 мес после начала комбинированной ИСТ.</p><p><bold>Результаты. </bold>Разнонаправленные изменения соотношения субпопуляций Т-клеток до начала ИСТ были обнаружены у всех больных АА, включенных в проспективное исследование: у большинства больных выявлялась большая доля эффекторных CD4<sup>+</sup>- и CD8<sup>+</sup>-Т-клеток (у 61 и 83 % больных соответственно), СD4<sup>+</sup>-Т-клеток памяти (у 63 % больных) и меньшая доля наивных CD4<sup>+</sup>- и CD8<sup>+</sup>-Т-клеток (у 81 и 51 % больных соответственно) по сравнению с донорами (<italic>p </italic>&lt;0,05). Нарушения соотношения субпопуляций Т-клеток, обусловливающие аномальный иммунный ответ, наиболее выражены при сверхтяжелой форме АА. При ответе на лечение у больных не обнаружено достоверных изменений в субпопуляционном составе Т-клеток ни до начала ИСТ, ни через 6 мес после по сравнению с донорами. При отсутствии ответа на лечение выявлена большая доля эффекторных CD4<sup>+</sup>- и CD8<sup>+</sup>-Т-клеток, СD4<sup>+</sup>-Т-клеток памяти и меньшая доля наивных CD4<sup>+</sup>- и CD8<sup>+</sup>-Т-клеток по сравнению с донорами уже до начала ИСТ (<italic>p </italic>&lt;0,05). Из всех цитометрических параметров была получена достоверная связь между динамикой эффекторных CD8<sup>+</sup>-Т-клеток и ответом на лечение (<italic>р </italic>= 0,040). Количество этих клеток в дебюте и контрольных точках достоверно коррелировало с ответом на ИСТ. У больных, у которых не получено ответа на лечение к 3-му месяцу от начала ИСТ, происходит дальнейший рост количества эффекторных CD8<sup>+</sup>-Т-клеток, несмотря на продолжение ИСТ, что диктует необходимость усиления и интенсификации ИСТ в кратчайшие сроки (между 3-м и 6-м месяцами) у этих больных, а именно проведения 2-го курса лошадиным антитимоцитарным глобулином и продолжения терапии циклоспорином либо рассмотрения в частном порядке вопроса об альтернативном лечении (аллогенной трансплантации гемопоэтических стволовых клеток).</p><p><bold>Заключение. </bold>Изучение динамики субпопуляционного состава Т-клеток, в частности эффекторных Т-клеток, позволяет определиться с тактикой дальнейшего лечения на ранних сроках ИСТ в целях подбора оптимальной программы лечения для каждого больного АА.</p></trans-abstract><kwd-group xml:lang="en"><kwd>aplastic anemia</kwd><kwd>bone marrow failure</kwd><kwd>immunosuppressive therapy</kwd><kwd>T cell subpopulation</kwd><kwd>flow cytometry</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>апластическая анемия</kwd><kwd>костномозговая недостаточность</kwd><kwd>иммуносупрессивная терапия</kwd><kwd>субпопуляция Т-клеток</kwd><kwd>проточная цитометрия</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Bacigalupo A.
