ALCL is characterized by a heterogeneous clinical manifestation with nodal and extranodal lesions, various morphological variants, variable expression of T-cell markers and cytogenetic aberrations involving the <italic>ALK </italic>gene. Due to the rarity of this disease (up to 10–15 % of all non-Hodgkin’s lymphomas in children), there are no uniform standards for ALCL therapy. Block treatment schemes similar to those applied in the treatment of B-cell non-Hodgkin’s lymphomas are used; less often there can be used protocols similar to the therapy of acute lymphoblastic leukemia. Taking into account the unfavorable effect on the prognosis of T-cell markers expression in ALCL, we developed protocol ALCL-NII DOG-2003, which considered the immunological features of the tumor.</p><p><bold>Aim. </bold>To assess the effectiveness of differentiated immunobased ALCL therapy by domestic protocol ALCL-NII DOG-2003.</p><p><bold>Materials and methods. </bold>From 01.03.1997 to 01.10.2022, 60 patients with primary diagnosed ALCL were included in the study. The diagnosis was based on histological, immunohistochemical and cytogenetic criteria of WHO classification of hematopoietic and lymphoid tissues tumors. According to treatment program, patients were subdivided in 2 groups: patients treated by NHL-BFM 95 protocol (<italic>n </italic>= 21) and by ALCL-NII DOG-2003 (<italic>n </italic>= 39). The male:female ratio was 1,3:1 in group of patients with NHL-BFM 95 protocol and 1,4:1 – in ALCL-NII DOG 2003. The effectiveness assessment was based on overall, event-free and relapse-free survival with SPSS 21.0 computed program. The differences between groups were significant with <italic>p </italic>≤0.05.</p><p><bold>Results. </bold>The use of risk-adopted, immuno-oriented protocol made it possible to obtain a 10‑year EFS in 97.4 ± 2.6 %, whereas results of the standard NHL-BFM 95 protocol are 63.9 ± 10 % (<italic>p </italic>= 0.001).</p><p><bold>Conclusion. </bold>The use of a differentiated approach to ALCL treatment, considering not only the stage and prognostic risk group, but also the immunological features of the tumor, is more effective, and allows achieving significantly higher survival rates, than standard program (NHL-BFM 95).</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение. </bold>Анапластическая крупноклеточная лимфома (АККЛ) является редким вариантом высокоагрессивных лимфом у детей и подростков. Для АККЛ характерны гетерогенная клиническая картина с поражением нодальных и экстранодальных зон, различные морфологические варианты, вариабельная экспрессия Т-клеточных маркеров и цитогенетические аберрации с вовлечением гена <italic>ALK</italic>. В связи с редкостью этого заболевания (до 10–15 % всех неходжкинских лимфом у детей) единые стандарты терапии АККЛ не разработаны. Применяются блоковые схемы лечения, аналогичные используемым в терапии В-клеточных неходжкинских лимфом, реже – протоколы, напоминающие таковые при остром лимфобластном лейкозе. С учетом неблагоприятного влияния на прогноз экспрессии Т-клеточных маркеров при АККЛ был разработан протокол лечения АККЛ-НИИ ДОГ-2003, который учитывает иммунологические особенности опухоли.</p><p><bold>Цель исследования </bold>– оценить эффективность дифференцированной, иммуноориентированной терапии АККЛ по отечественному протоколу АККЛ-НИИ ДОГ-2003.</p><p><bold>Материалы и методы. </bold>С 01.03.1997 по 01.10.2022 в исследование были включены 60 пациентов с впервые установленным диагнозом АККЛ. Верификация диагноза проводилась путем гистологического, иммуногистохимического и цитогенетического исследований опухолевой ткани и соответствовала критериям классификации опухолей кроветворной и лимфоидной тканей Всемирной организации здравоохранения. В зависимости от программы терапии были выделены 2 группы: пациенты, которым проводилась терапия по протоколу NHL-BFM 95 (<italic>n </italic>= 21) и по протоколу АККЛ-НИИ ДОГ-2003 (<italic>n </italic>= 39). Соотношение мальчиков и девочек, получавших терапию по протоколам NHL-BFM 95 и АККЛ -НИИ ДОГ-2003, составило 1,3:1 и 1,4:1 соответственно. Оценку эффективности терапии проводили путем анализа общей, безрецидивной и бессобытийной выживаемости с использованием программы SPSS 21.0. Различия между группами считали достоверными при <italic>р </italic>≤0,05.</p><p><bold>Результаты. </bold>Риск-адаптированный, иммуноориентированный подход позволил добиться 10‑летней бессобытийной выживаемости у 97,4 ± 2,6 % больных, тогда как при лечении по стандартному протоколу NHL-BFM 95 данный показатель составил 63,9 ± 10 % (<italic>р </italic>= 0,001).</p><p><bold>Заключение. </bold>Дифференцированный подход к терапии АККЛ, учитывающий не только стадию и прогностическую группу риска, но и иммунологические особенности опухоли, является более эффективным, позволяющим достичь достоверно более высоких показателей выживаемости больных, чем стандартные программы (NHL-BFM 95).</p></trans-abstract><kwd-group xml:lang="en"><kwd>anaplastic large cell lymphoma</kwd><kwd>treatment</kwd><kwd>children</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>анапластическая крупноклеточная лимфома</kwd><kwd>лечение</kwd><kwd>дети</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Valiev T.T., Popa A.V., Levashov A.S. et al.
