Actually, treatment results of Hodgkin lymphoma (HL) are the most dramatic oncohematology achievements, therefore modern treatment protocols designed to toxicity reduction with the same high level of patients’ survival. Time of complete response occupies a central position in the prognostic factors for HL and helps to find a group of patients whose treatment could be de-escalated.</p><p><bold>Objective</bold>: to evaluate the efficacy of original domestic risk-adopted protocol RDC POG-HL 2003 with treatment de-escalation and refused radiation therapy (RT) for early-responded patients.</p><p><bold>Materials and methods</bold>. 192 patients were enrolled in prospective RDC POG-HL 2003 protocol from February 2003 to November 2020. Median age was 12.8 years (from 3 to 17). Local stages (IA–IIA) were diagnosed in 48 (25 %) patients, disseminated (IIB–IVB) – in 144 (75 %) cases. For local (IA–IIA) stages by RCD POG-HL 2003 treatment included DBVE + RT, for disseminated (IIB–IVB) – BEACOPP escalated (esc.) + RT. In case of 70 % and more tumor reduction after 4 induction courses of BEACOPP-esc., the following treatment included less intensive schemes (ABVD, COPP/ABV). Because of high risk of breast cancer in girls after mediastinal RT, it was possible to omit a RT in case of early response.</p><p><bold>Results</bold>. All patients with local stages are alive by the time of study end. Event- and relapse-free survivals in this group were 97.8 ± 2.5 % (median follow up 181.9 ± 4.8 months). Event-free survival for disseminated stages patients was 90.3 ± 3.3 % (median follow up 179.1 ± 4.2 months), relapse-free survival – 93.5 ± 2.1 % (median follow up 191.7 ± 2.3 months) and overall survival – 97.9 ± 1.2 % (median follow up 196.3 ± 2.6 months). In 48 (25 %) patients it was possible to omit RT without reducing survival rates.</p><p><bold>Conclusion</bold>. Differentiated HL treatment with respect to disease stage and time of complete response is a key to success of treatment. Such approach permits us to reduce cumulative therapy toxicity by its de-escalation and, in some cases, to omit RT.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение</bold>. В настоящее время результаты терапии лимфомы Ходжкина (ЛХ) являются наиболее впечатляющими достижениями онкогематологии, в связи с чем современные протоколы лечения направлены на снижение токсичности при сохранении высоких показателей излечения больных. Центральное место среди факторов прогноза при ЛХ занимает время достижения полного противоопухолевого эффекта, что позволяет выделить группу больных, которым возможна дезэскалация терапии.</p><p><bold>Цель исследования</bold> – оценка эффективности оригинального отечественного риск-адаптированного протокола НИИ ДОГ-ЛХ 2003, предполагающего дезэскалацию терапии и отказ от лучевой терапии (ЛТ) в группе больных с ранним противоопухолевым эффектом.</p><p><bold>Материалы и методы</bold>. В период с февраля 2003 г. по ноябрь 2020 г. в протокол НИИ ДОГ-ЛХ 2003 были включены 192 пациента. Средний возраст больных составил 12,8 (3–17) года. Локальные (IA–IIA) стадии были диагностированы у 48 (25 %) больных, распространенные (IIB–IVB) – у 144 (75 %). При локальных стадиях протокол НИИ ДОГ-ЛХ 2003 предполагает терапию по схеме DBVE + ЛТ, тогда как при распространенных – BEACOPP-эскалированный (BEACOPP-эск.) + ЛТ. В случае сокращения размера опухоли более чем на 70 % после 4 индукционных курсов BEACOPP-эск. дальнейшее лечение проводили по менее интенсивным схемам (ABVD, COPP/ABV). Кроме этого, в связи с высоким риском развития рака молочной железы при выполнении ЛТ на область средостения у девочек при достижении раннего противоопухолевого эффекта ЛТ им не проводилась.</p><p><bold>Результаты</bold>. К моменту окончания исследования все больные с локальными стадиями живы. Бессобытийная и безрецидивная выживаемость в данной группе составила 97,8 ± 2,5 % (медиана наблюдения 181,9 ± 4,8 мес). При распространенных стадиях ЛХ бессобытийная выживаемость оказалась 90,3 ± 3,3 % (медиана наблюдения 179,1 ± 4,2 мес), безрецидивная выживаемость – 93,5 ± 2,1 % (медиана наблюдения 191,7 ± 2,3 мес), общая выживаемость – 97,9 ± 1,2 % (медиана наблюдения 196,3 ± 2,6 мес). У 48 (25 %) пациентов удалось отказаться от проведения ЛТ без снижения показателей выживаемости.</p><p><bold>Заключение</bold>. Дифференцированная терапия ЛХ с учетом стадии опухолевого процесса и сроков достижения противоопухолевого эффекта является залогом успешного лечения больных.
