The prospective study included 138 patients with enlarged superficial lymph nodes (LN). Based on a previous histological examination, patients were divided into two groups: 1st group (n = 108) – patients with non-Hodgkin’s lymphomas and Hodgkin’s lymphoma; 2nd group (n = 30) – patients with metastasis of solid tumors in superficial LN. All patients underwent ultrasound elastography of the enlarged LN using ARFI technology. In VTI study the Area Ratio parameter was evaluated, and the minimum and average values of the shear wave velocity were estimated in VTQ study.</p><p><bold>Results</bold>. According to the results of VTI study the Area Ratio parameter for enlarged LN in lymphoma (1st group) and for metastatic lymphadenopathy (2 nd group) were 1.031 ± 0.197 and 0.851 ± 0.15, respectively (p = 0.000009). The cut off value of the Area Ratio parameter was 0.901 with sensitivity, specificity and accuracy 80.6, 70.0 and 78.8 %, respectively. Minimum values of shear wave velocities for 1st and 2 nd groups were 1.980 ± 0.557 and 2.214 ± 0.367 m/s, respectively (p = 0.032). The cut off values of the average shear wave velocity in the differentiating of lymphomatous and metastatic lymphadenopathy are determined at the level of 2.00 m/s, with sensitivity of 70.0 %, specificity of 59.3 %, and accuracy of 61.6 %.</p><p><bold>Conclusion</bold>. Ultrasound elastography with ARFI technology demonstrated statistically significant differences in the Area Ratio parameter and in the minimum shear wave velocity in the enlarged superficial LN in lymphoma and with metastasis that can be used as a preliminary non-invasive differential diagnosis of enlarged superficial LN in these conditions. Moreover, the Area Ratio parameter has a statistically more significant effect on differentiating of lymphomatous and metastatic lymphadenopathy.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель исследования</bold> – определить возможности технологии ARFI (Acoustic Radiation Force Impulse), представленной приложениями Virtual Touch™ Tissue Imaging (VTI) и Virtual Touch™ Tissue Quantification (VTQ), в дифференциальной диагностике лимфопролиферативных и метастатических изменений поверхностных лимфатических узлов (ЛУ).</p><p><bold>Материалы и методы</bold>. В проспективное исследование включены 138 пациентов с увеличенными поверхностными ЛУ. На основании ранее проведенного гистологического исследования пациенты были разделены на 2 группы: 1-я (n = 108) – пациенты с неходжкинскими лимфомами и лимфомой Ходжкина; 2-я (n = 30) – пациенты с метастазами солидных (эпителиальных) опухолей в поверхностных ЛУ. Всем пациентам выполнена ультразвуковая эластография измененных ЛУ с применением технологии ARFI. При исследовании VTI оценивали параметр Area Ratio (соотношение площадей), при VTQ – минимальные и средние значения скорости сдвиговой волны.</p><p><bold>Результаты</bold>. По результатам исследования VTI параметры Area Ratio для измененных ЛУ при лимфоме (1-я группа) и при метастатическом поражении (2-я группа) составили 1,031 ± 0,197 и 0,851 ± 0,15 соответственно (p = 0,000009). Чувствительность, специфичность и точность при пороговом значении Area Ratio на уровне 0,901 составили 80,6; 70,0 и 78,8 % соответственно. Минимальные значения скорости сдвиговой волны у пациентов 1-й и 2-й групп достоверно различались: 1,980 ± 0,557 и 2,214 ± 0,367 м/с соответственно (p = 0,032). Пороговое значение минимальной скорости сдвиговой волны в дифференциальной диагностике лимфомы и метастазов определено на уровне 2,00 м/с с чувствительностью 70,0 %, специфичностью 59,3 % и точностью 61,6 %.</p><p><bold>Заключение</bold>. Ультразвуковая эластография с использованием технологии ARFI продемонстрировала статистически достоверные различия в показателях параметра Area Ratio (при VTI) и минимальной скорости сдвиговой волны (при VTQ) в измененных поверхностных ЛУ при лимфоме и метастазах, что может быть использовано в качестве предварительной неинвазивной дифференциальной диагностики.
