These include pomalidomide, a 3 </italic>rd generation immunomodulator capable of treating double refractory multiple myeloma (to lenalidomide and bortezomib). Efficacy and safety of pomalidomide combined with low doses of dexamethasone have been<italic> established in MM-003 and STRATUS trials. The summary presents the data on opportunities to further enhance the efficacy of pomalidomide combined with other antitumor drugs in patients with relapsed and refractory multiple myeloma who previously received 4–5 lines of therapy. It has been shown that triplets based on pomalidomide and dexamethasone combined with cyclophosphamide, bendamustine, daratumumab, carfilzomib, elotuzumab are highly effective in double refractory multiple myeloma patients. A combination of pomalidomide with proteasome inhibitors is a promising treatment provided that there is no refractoriness to bortezomib.</italic></p></abstract><trans-abstract xml:lang="ru"><p>В настоящее время существенно расширились возможности в лечении рефрактерной/рецидивирующей множественной миеломы<italic> ввиду появления новых препаратов таргетного действия, одним из которых является помалидомид. Помалидомид – иммуномодулирирующий препарат 3-го поколения, позволяющий преодолеть двойную рефрактерность (к леналидомиду и бортезомибу). Эффективность и безопасность помалидомида с низкими дозами дексаметазона подтверждены в исследованиях ММ-003 и STRATUS. В обзоре представлены данные о возможности повышения эффективности лечения при комбинации помалидомида с другими противоопухолевыми препаратами у больных рефрактерной/рецидивирующей множественной миеломой, которые ранее получили 4–5 линий терапии. Показано, что триплеты на основе помалидомида и дексаметазона в сочетании с циклофосфамидом, бендамустином, даратумумабом, карфилзомибом, элотузумабом высокоэффективны у пациентов с двойной рефрактерностью.
