Infections occurred in 242 (50 %) of CT cycles and predominated in neutropenic compared to non-neutropenic patients (80 % vs 6 %, p &lt;0.0001). Infections prevailed in patients with AML compared to ALL patients (93 % vs 18 %, p &lt;0.0001) as in patients with neutropenia (96 % vs 45 %, p &lt;0.0001) and without neutropenia (27 % vs 4 %, p = 0.02). Prolongation of neutropenia from 1–7 days to ≥22 days was associated with increase of infections rate from 52 to 96 % (p &lt;0.0001). Incidence of infections in AML patients was high (92–100 %) regardless of neutropenia duration, whereas in ALL patients it increased from 25–33 to 91 % if neutropenia lengthened from 2 weeks to ≥22 days. During neutropenia the probability of fever of unknown origin was 33.9 %, clinically documented infection – 31.3 %, bacteremia – 17.2 %. They predominated in the first 2 weeks of neutropenia. Probability of invasive aspergillosis (IA) increased after 28 days of neutropenia and reached 66 % on the 55th day. First case of IA in patients with ALL was on 28th day of neutropenia whilst in AML patients – 4 (44 %) of 9 occurred more early (6–16 days of neutropenia). Nine (6 %) of 110 patients died, 4 (4 %) of them due to infection.</p><p><bold>Conclusions.</bold> Neutropenia was a predictor of infectious complications in patients with AML and ALL. Correlation between duration of neutropenia and incidence of infections was in patients with ALL, whereas in AML patients the rate of infections was high regardless of neutropenia duration. In patients with neutropenia for 2 weeks the most common types of infection were fever of unknown origin, clinically documented infection and bacteremia whilst IA predominated if neutropenia duration was ≥28 days.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> При реализации программной химиотерапии (ХТ) у больных гемобластозами регистрируется высокая частота инфекционных осложнений, характер которых определяется рядом факторов, включая гранулоцитопению. <bold>Цель исследования</bold> – изучить частоту и характер инфекций у больных de novo острыми миелоидными лейкозами (ОМЛ) и острыми лимфобластными лейкозами (ОЛЛ) в зависимости от длительности гранулоцитопении при применении современных программ ХТ.<bold>Материалы и методы.</bold> Проспективное исследование (2013–2015 гг.) включало 110 больных (66 ОМЛ, 44 ОЛЛ), которым было проведено 480 курсов ХТ за 6 мес.<bold>Результаты.</bold> Гранулоцитопения с медианой длительности 15 (2–55) дней была в 288 (60 %) курсах ХТ. Инфекционные осложнения возникли в 242 (50 %) курсах ХТ и превалировали у больных с гранулоцитопенией в сравнении с больными без гранулоцитопении (80 % против 6 %, p &lt;0,0001). Развитие инфекций наблюдалось чаще у больных ОМЛ, чем у больных ОЛЛ (93 % против 18 %,p &lt;0,0001), как при наличии гранулоцитопении (96 % против 45 %, p &lt;0,0001), так и при ее отсутствии (27 % против 4 %,p = 0,02). При удлинении гранулоцитопении от 1–7 до 22 дней и более доля инфекций возрастала с 52 до 96 % (p &lt;0,0001). Частота инфекционных осложнений у больных ОМЛ была высокой (92–100 %) при любой длительности гранулоцитопении, а у больных ОЛЛ увеличивалась с 25–33 до 91 % при удлинении гранулоцитопении с 2 нед до 22 дней и более. В период гранулоцитопениивероятность развития лихорадки неясной этиологии составила 33,9 %, клинически доказанной инфекции – 31,3 %, бактериемии – 17,2 %. Регистрация их наблюдалась в основном в первые 2 нед гранулоцитопении. Вероятность развития инвазивногоаспергиллеза (ИА) возрастала после 28-го дня гранулоцитопении и достигла 66 % к 55-му дню. Первый случай ИА у больных ОЛЛбыл на 28-й день гранулоцитопении, а у больных ОМЛ – 4 (44 %) из 9 в более ранние сроки (6–16-й дни гранулоцитопении). Летальные исходы были у 7 (6 %) из 110 больных, из них у 4 (4 %) по причине инфекции.<bold>Выводы.</bold> Гранулоцитопения являлась предиктором развития инфекционных осложнений у больных ОМЛ и ОЛЛ. Корреляция длительности гранулоцитопении и частоты инфекций была выявлена у больных ОЛЛ, в то время как у больных ОМЛ частота инфекционных осложнений была высокой вне зависимости от длительности гранулоцитопении.
