At the first onset of the disease plasmacytoma was diagnosed in 21 patients: bone plasmacytoma, associated with skeletal bones – in 14 patients; extramedullary plasmacytoma, emerging in various organs not connected with bone tissue – in 7 patients. All patients underwent BM trephine biopsy and plasmacytoma biopsy with subsequent histological examination. BM and plasmacytoma histological specimens were studied using LEICA DM4000B microscope. Frequency domain analysis (cross tables, Fisher–Freeman test) was used for data statistical analysis.<bold/></italic></p><p><bold><italic>Results. </italic></bold><italic>The analysis showed that the histological features of BM in MM patients with extramedullary plasmacytoma statistically significantly differed from that in MM patients with bone plasmacytoma and without plasmacytoma. As a result of the analysis, the relationship between BM morphological variant and tumor advancement became apparent. When comparing the morphological pattern of the bone and extramedullary plasmacytomas, no significant differences were found, however, the substrate of the extramedullary plasmacytoma was more often represented by tumor cells with immature morphology as compared to the substrate of the bone plasmacytoma.<bold/></italic></p><p><bold><italic>Conclusion. </italic></bold><italic>The established differences in the histological structure of the BM in MM patients with extramedullary plasmacytoma suggest that this type of the disease stands apart and requires further detailed pathomorphological study.<bold/></italic></p></abstract><trans-abstract xml:lang="ru"><p><bold><italic>Цель исследования. </italic></bold><italic>Изучение гистологического строения опухолевого субстрата костного мозга (КМ) и плазмоцитомы у больных множественной миеломой (ММ).<bold/></italic></p><p><bold><italic>Материалы и методы. </italic></bold><italic>В исследование включено 35 пациентов (19 мужчин и 16 женщин) в возрасте от 23 до 73 лет с впервые диагностированной ММ. В дебюте заболевания плазмоцитома была диагностирована у 21 больного: костная, ассоциированная с костями скелета – у 14 больных, экстрамедуллярная, формирующаяся в различных органах, не связанных с костной тканью, – у 7 пациентов. Всем больным была выполнена трепанобиопсия КМ и биопсия плазмоцитомы с последующим гистологическим исследованием. Просмотр гистологических препаратов КМ и плазмоцитомы осуществлен с использованием микроскопа LEICA DM4000B. При статистическом анализе данных использован частотный анализ (таблицы сопряженности, критерий Фишера – Фримана).<bold/></italic></p><p><bold><italic>Результаты. </italic></bold><italic>Проведенный анализ показал, что у больных ММ с экстрамедуллярной плазмоцитомой морфологический вариант КМ статистически значимо отличался от такового у больных ММ с костной плазмоцитомой и без плазмоцитомы. В результате проведенного анализа стала очевидной взаимосвязь между морфологическим вариантом КМ и распространенностью опухолевого процесса. При сопоставлении морфологической картины костной и экстрамедуллярной плазмоцитомы достоверных различий не получено, тем не менее субстрат экстрамедуллярной плазмоцитомы несколько чаще был представлен опухолевыми клетками с незрелой морфологией по сравнению с субстратом костной плазмоцитомы.<bold/></italic></p><p><bold><italic>Заключение. </italic></bold><italic>Доказанные различия в гистологическом строении КМ больных ММ с экстрамедуллярной плазмоцитомой свидетельствуют о том, что данная форма болезни стоит обособленно и требует дальнейшего детального изучения с позиций патоморфологии.<bold/></italic></p></trans-abstract><kwd-group xml:lang="en"><kwd>multiple myeloma</kwd><kwd>plasmacytoma</kwd><kwd>bone marrow</kwd><kwd>trephine biopsy</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>множественная миелома</kwd><kwd>плазмоцитома</kwd><kwd>костный мозг</kwd><kwd>трепанобиопсия</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">1.
