An accepted fact in allogeneic hematopoietic stem cells transplantation (allo-HSCT) from unrelated donors is that matching for HLA genes is critical to ensure the best outcomes for patients. The current gold standard is an unrelated donor matched for 10/10 HLA alleles (HLA-A, -B, -C, -DRB1, -DQB1). In the HLA-mismatched setting graft-versus-host disease (GVHD) is increased, and overall survival becomes significantly worse. The importance of HLA-DPB1matching is more controversial.</p><p><bold>The aim of our study </bold>was to evaluate the impact of HLA-DPB1 mismatches on outcome of patients who underwent 10/10 HLA matched unrelated HSCT.</p><p><bold>Materials and methods. </bold>49 patients treated with allo-HSCT in transplant center of National Research Center for Hematology from 10/10 HLA-matched unrelated donors were included in the study. High-resolution typing for HLA-DPB1 was done by PCR with sequence specific primers (SSP) using Olerup typing kits. HLA-DPB1 permissive/nonpermissive mismatches were examined according to TCE groups. Overall survival, event-free survival and survival without acute graft-versus-host disease were calculated by Kaplan–Meier method, and compared with log-rank test. Proportional hazard Cox models were used for multivariate analysis.</p><p><bold>Results. </bold>The 3-year overall survival after allo-HSCT was 68 %, event-free survival – 51 %, acute GVHD-free survival – 62 %. No significant impacts of HLA-DPB1 disparities on overall, event-free survival and probability of acute graft-versus-host disease were observed. Patients with nonpermissive HLA-DPB1-incompatible donors had a tendency to increased event-free survival. The factors significantly increasing risk of acute GVHD were peripheral blood grafts, advanced-stage disease and male gender of recipients.</p><p><bold>Conclusion</bold><bold>. </bold>The factors associated with acute GVHD are peripheral blood grafts, advanced-stage disease and male sex of recipients. For more precise evaluation of impact of HLA-DPB1-incompatibility on results of allo-HSCT further investigations are needed.</p></abstract><trans-abstract xml:lang="ru"><p/><p><bold>Введение</bold>. Совместимость донора и реципиента по HLA-A, -B, -C, -DRB1, -DQB1-генам является «золотым стандартом» при подборе неродственного донора для трансплантации аллогенных гемопоэтических стволовых клеток (аллоТГСК), так как несовпадение повышает частоту реакции «трансплантат против хозяина» (РТПХ) и ухудшает выживаемость, однако значение расхождения по аллелям гена HLA-DPB1 остается неопределенным.</p><p><bold>Цель исследования </bold>– оценка влияния расхождения по аллелям гена HLA-DPB1 на исход аллоТГСК от HLA-A-B-C–DRB1-DQB1совместимого неродственного донора.</p><p><bold>Материалы и методы</bold>. В исследовании приняли участие 49 больных с различными гемобластозами, которым в отделении трансплантации костного мозга ФГБУ ГНЦ МЗ РФ было выполнено 52 аллоТГСК от HLA-A-B-C–DRB1-DQB1-совместимого неродственного донора. Высокоразрешающее типирование гена HLA-DPB1 проводили методом полимеразной цепной реакции с сиквенсспецифическими праймерами у пар донор – реципиент, которые совпали по вариантам генов HLA-A-B-C–DRB1-DQB1 при типировании с высоким разрешением. Общая выживаемость (ОВ), бессобытийная выживаемость (БСВ), выживаемость без острой РТПХ (оРТПХ) рассчитаны по методу Каплана–Майера. Для оценки статистической значимости различий выживаемости использовали log-ранг-тест. Многофакторный анализ проведен с применением регрессионной модели Кокса.</p><p><bold>Результаты</bold>. Трехлетняя ОВ после аллоТГСК составила 68 %, 3-летняя БСВ – 51 %, 3-летняя выживаемость без оРТПХ – 62 %. Несовпадение донора и больного по DPB1-аллелям не оказывало статистически значимого влияния на ОВ, БСВ и повышение вероятности развития оРТПХ после аллоТГСК; однако у больных с аллоТГСК от донора с непермиссивным несовпадением по DPB1аллелям наблюдалась тенденция к повышению БСВ. Факторами риска развития оРТПХ после аллоТГСК от неродственного HLA-A-B-C–DRB1-DQB1-совместимого донора были трансплантация стволовых клеток периферической крови, продвинутая стадия основного заболевания и мужской пол реципиента.</p><bold>Заключение</bold>. Факторы риска развития оРТПХ после аллоТГСК от неродственного HLA-A-B-C–DRB1-DQB1-совместимого донора – трансплантация стволовых клеток периферической крови; продвинутая стадия основного заболевания, мужской пол реципиента.
