The median age was 69 years. The majority of patients were in the older age group (60.4 %), with advanced disease stages (70 %), non-GCB subtypes (60 %), and high risk according to the international prognostic index (67 %). Of all patients included, 62.1 % received immunochemotherapy according to the R-CHOP program (rituximab, cyclophosphamide, doxorubicin, vincristine, prednisolone), 12.6 % received R-DA-EPOCH (rituximab, etoposide, prednisolone, vincristine, cyclophosphamide, doxorubicin), 2.6 % received intensive regimens, 14.7 % received R-miniCHOP (rituximab, cyclophosphamide, doxorubicin, vincristine, prednisolone in reduced doses), 7.8 % received RB (rituximab, bendamustine) or R-CVP (rituximab, cyclophosphamide, vincristine, prednisolone).</p> <p><bold>Results.</bold> In the overall patient cohort (<italic>n</italic> = 1060), the objective response rate was 79.7 % (<italic>n</italic> = 845). Complete metabolic response was achieved by 66.4 % (<italic>n</italic> = 704) of patients. The complete response rate varied depending on the treatment regimen: in the R-CHOP group it was 68.4 % (<italic>n</italic> = 451), when using R-DA-EPOCH – 69.4 % (<italic>n</italic> = 93), intensive regimens – 89.3 % (<italic>n</italic> = 25). Among patients receiving R-miniCHOP, complete response was achieved in 62.2 % (<italic>n</italic> = 97) of cases, while in the RB / R-CVP group this rate was the lowest and amounted to 45.8 % (<italic>n</italic> = 38). With a median follow-up of 24 months, the 2-year overall survival in the total group was 70 % (95 % confidence interval 67–73), and the 2-year progression-free survival was 44 % (95 % confidence interval 40–48).</p> <p><bold>Conclusion. </bold>Results of first-line DLBCL therapy efficacy analysis indicate unsatisfactory treatment outcomes, particularly in the high-risk group for early progression. We suggest that biologically targeted antitumor therapy based on the identified tumor genotype represents the most promising clinical approach for patients with newly diagnosed DLBCL.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель исследования</bold> – оценка структуры и эффективности терапии впервые диагностированной диффузной В-крупноклеточной лимфомы (ДВКЛ) у взрослых в рамках реальной клинической практики по данным Московского онкологического регистра.</p> <p><bold>Материалы и методы.</bold> Ретроспективно проанализированы данные 1060 пациентов с впервые диагностированной ДВКЛ из Московского онкологического регистра, которые получали лечение с 2022 по 2025 г. в рамках практического здравоохранения г. Москвы. Медиана возраста составила 69 лет. Превалировали пациенты старшей возрастной группы (60,4 %), с распространенными стадиями заболевания (70 %), non-GCB-подтипами (60 %), высоким риском по международному прогностическому индексу (67 %). Из всех включенных в исследование пациентов иммунохимиотерапию по программе R-CHOP (ритуксимаб, циклофосфамид, доксорубицин, винкристин, преднизолон) получили 62,1 %; R-DA-EPOCH (ритуксимаб, этопозид, преднизолон, винкристин, циклофосфамид, доксорубицин) – 12,6 %; интенсивные схемы – 2,6 %; R-miniCHOP (ритуксимаб, циклофосфамид, доксорубицин, винкристин, преднизолон в редуцированных дозах) – 14,7 %; RB (ритуксимаб, бендамустин) или R-CVP (ритуксимаб, циклофосфамид, винкристин, преднизолон) – 7,8 %.</p> <p><bold>Результаты.</bold> В общей когорте пациентов (<italic>n</italic> = 1060) частота объективного ответа составила 79,7 % (<italic>n</italic> = 845). Полного метаболического ответа достигли 66,4 % (<italic>n</italic> = 704) больных. Частота полных ответов варьировала в зависимости от режима терапии: в группе R-CHOP она составила 68,4 % (<italic>n</italic> = 451), при использовании R-DA-EPOCH – 69,4 % (<italic>n</italic> = 93), на фоне интенсивных схем – 89,3 % (<italic>n</italic> = 25). Среди пациентов, получавших R-miniCHOP, полный ответ достигнут в 62,2 % (<italic>n</italic> = 97) случаев, тогда как в группе RB / R-CVP этот показатель был наименьшим и составил 45,8 % (<italic>n</italic> = 38). При медиане времени наблюдения 24 мес 2-летняя общая выживаемость в общей группе составила 70 % (95 % доверительный интервал 67–73), 2-летняя выживаемость без прогрессирования – 44 % (95 % доверительный интервал 40–48).</p> <p><bold>Заключение.</bold> Результаты анализа эффективности 1-й линии терапии ДВКЛ свидетельствуют о неудовлетворительных результатах лечения, особенно в группе высокого риска раннего прогрессирования.
