Netupitant / palonosetron was administered at a dose of 1 capsule (300 mg netupitant; 0.5 mg palonosetron) 1 h before the start of chemotherapy conditioning (day –3) and dexamethasone was administered at day – 3 (12 mg), day – 2…0 (8 mg). Intravenous metoclopramide 10 mg was used as a rescue antiemetic when needed. Patients were evaluated every day for up to 12 days after autologous hematopoietic stem cell transplantation. The primary endpoint was the rate of complete response (no emesis and no rescue medication during 120 h after conditioning). Secondary endpoints were defined as the rate of complete response during the acute phase (0–24 h) and delayed phase (25–120 h) from the start of high‑dose melphalan. The safety of combination was also evaluated. 20 patients were included. The median age was 53.5 (37–67) years.</p><p><bold>   Results.</bold> Netupitant / palonosetron with dexamethasone prophylaxis showed a high efficacy in preventing chemotherapy‑induced nausea and vomiting during the high‑dose melphalan conditioning period. The primary outcome was largely achieved with a complete response observed in 14 (70 %) patients at 120 h. The complete response rate during the acute phase was 70 % (n = 14). during the delayed phase, the complete response rate was 95 % (n = 19). Grade 1 nausea and vomiting were experienced by 70 % of the patients, grade 2 – by 30 % of patients. No patients had grade 3–4 nausea or vomiting. Intravenous metoclopramide 10 mg was used in 30 % (6 patients).</p><p><bold>   Conclusion. </bold>Firstly in Russia we assess efficacy of netupitant / palonosetron with dexamethasone for chemotherapy‑induced nausea and vomiting in 20 patients with multiple myeloma who received high‑dose melphalan for autologous hematopoietic stem cell transplantation.</p></abstract><trans-abstract xml:lang="ru"><p><bold>   Цель исследования</bold> – оценить способность нетупитанта / палоносетрона в комбинации с дексаметазоном предотвращать развитие тошноты и рвоты у пациентов с множественной миеломой, которым запланировано выполнение аутологичной трансплантации гемопоэтических стволовых клеток с мелфаланом в качестве предтрансплантационного кондиционирования.</p><p><bold>   Материалы и методы. </bold>Пациенты старше 18 лет, которым диагностирована множественная миелома и запланировано выполнение аутологичной трансплантации гемопоэтических стволовых клеток в соответствии с критериями Международной рабочей группы, получили профилактику тошноты и рвоты с помощью комбинации нетупитанта / палоносетрона (однократно 300 мг нетупитанта и 0,5 мг палоносетрона за 1 ч до начала кондиционирования (– 3‑й день)) и дексаметазона (12 мг в –3‑й день и 8 мг в –2…0‑й дни). Метоклопрамид использовали при необходимости в качестве терапии «спасения». Эпизоды тошноты и рвоты регистрировали на протяжении 12 дней после инфузии гемопоэтических стволовых клеток. Выраженность тошноты и рвоты фиксировали с помощью цифровой оценочной шкалы согласно Общим терминологическим критериям для оценки нежелательных явлений (Common Terminology Criteria for Adverse Events, CTCAE) версии 4.03. Первичной конечной точкой была частота полного ответа (без эпизодов рвоты и потребности в терапии «спасения») в течение 120 ч после завершения кондиционирования. Вторичными конечными точками были частота полного ответа в острую (0–24 ч) и отстроченную (25–120 ч) фазы от начала введения мелфалана. Также оценивали профиль токсичности нетупитанта / палоносетрона в комбинации с дексаметазоном. В исследование включены 20 пациентов. Медиана возраста составила 53,5 (37–67) года.</p><p><bold>   Результаты. </bold>Профилактика тошноты и рвоты с помощью нетупитанта / палоносетрона в комбинации с дексаметазоном продемонстрировала высокую эффективность. Первичная конечная точка достигнута у 70 % пациентов в течение 120 ч после завершения кондиционирования. Полный ответ в острую фазу составил 70 % (14 пациентов), в отсроченную – 95 % (19 пациентов). Тошнота и рвота I степени отмечены у 70 % (14 пациентов), II степени – у 30 % (6 пациентов), III–Iv степени не зарегистрировано.
