The article describes a case of primary plasma cell leukemia in a 62‑year‑old patient who did not respond to the VCD induction program (bortezomib, cyclophosphamide, dexamethasone). Given the presence of t(11;14), a highly selective Bcl‑2 inhibitor, venetoclax, was included in the therapy regimen (ven‑vd regimen). After 3 cycles of venetoclax‑based induction, a very good partial remission was achieved. Then high‑dose MEL200 chemotherapy, autologous hematopoietic stem cell transplantation and 2 cycles of ven‑vd consolidation were implemented, achieving a complete response. MRD‑ negativity (10<sup>–5</sup>) was confirmed by flow cytometry on bone marrow aspirate at 2 time points. The remission duration at the time of publication was 24 months. The patient continues to receive maintenance therapy with lenalidomide.</p></abstract><trans-abstract xml:lang="ru"><p>   Плазмоклеточный лейкоз – редкое, агрессивное заболевание, характеризующееся клональной пролиферацией плазматических клеток, имеющее много общих черт как с плазмоклеточными новообразованиями, так и с острыми лейкозами. Несмотря на существенные изменения в подходах к терапии, прогноз при этом заболевании остается неблагоприятным. Приведено описание случая первичного плазмоклеточного лейкоза у пациентки 62 лет, которая не имела ответа на индукционную программу VCD (бортезомиб, циклофосфамид, дексаметазон). С учетом наличия t(11;14) в схему терапии включен высокоселективный ингибитор Bcl‑2 венетоклакс (схема ven‑vd). после 3 циклов индукции на основе венетоклакса достигнута очень хорошая частичная ремиссия. Далее реализованы высокодозная химиотерапия MEL200, аутологичная трансплантация гемопоэтических стволовых клеток и 2 цикла консолидации Ven‑Vd с достижением полного ответа. Отрицательный статус минимальной остаточной болезни (10<sup>–5</sup>) подтвержден методом проточной цитофлуориметрии по аспирату костного мозга в 2 временных точках.
