However, the flip side of high efficacy is its toxicity. Even adherence to modern guidelines for supportive care, including therapeutic drug monitoring, timely administration of the antidote (calcium folinate), maintaining alkaline blood and urine pH levels, and adequate hydration, does not always prevent the development of methotrexate‑induced organ toxicity. Therefore, further studies of the toxicity spectrum and the identification of potential prognostic factors for organ toxicity remains relevant.</p><p><bold>   Aim.</bold> To analyze the spectrum of immediate toxic effects of non‑Hodgkin’s lymphomas therapy with high‑dose MTX and to identify possible predictors of toxic events.</p><p><bold>   Materials and methods. </bold>From 2020 to 2024, 100 pediatric patients (under 18 years old) with primary diagnosed non‑Hodgkin’s lymphomas were enrolled the study and treated according to chemotherapy protocols (ALL IC‑bFM 2009, b‑NHL‑bFM 95, or ALCL‑NII dOG 2003) that included high‑dose MTX. The spectrum and severity of toxic events were analyzed with National Cancer Institute toxicity scales (uSA), and statistical analysis of feature distribution and correlation strength was performed.</p><p><bold>   Results.</bold> Hepatotoxicity of grades 3–4 was noted in 49 % of patients, nephrotoxicity developed in 1 % and corresponded to grade 3. Hematologic toxicity of grades 1–2 was observed in 8 % of patients, and grades 3–4 – in 92 %. Neurotoxicity developed in 9 % of patients (grade 1 – in 2 %, grade 2 – in 1 %, grade 3 – in 3 %, and grade 4 – in 3 %). Mucositis of grades 3–4 occurred in 30 % of patients. Infectious complications of grades 3–4 developed in 74 % of patients. No cases of treatment timing violation or toxicity‑related death were noted. Delayed methotrexate elimination beyond 54 hours was observed in 30 % of patients. Analyzing the influence of MTx dose on drug elimination and the incidence of toxic effects found that the more MTX dose, the higher incidence of delayed MTX elimination and toxic events incidence.</p><p><bold>   Conclusion. </bold>The study demonstrated the spectrum of methotrexate‑induced toxicity, including hematologic toxicity, mucositis, hepato‑, nephro‑, neurotoxicity, and infectious complications. delayed drug elimination correlates with increased MTx dose, which, in turn, associated with an increased risk of toxic events.</p></abstract><trans-abstract xml:lang="ru"><p><bold>   Введение. </bold>Метотрексат (МТX), применяемый в высоких дозах (1000–5000 мг/м<sup>2</sup>), зарекомендовал себя как один из ключевых компонентов успеха терапии неходжкинских лимфом. Обратной стороной высокой эффективности является токсичность. Даже соблюдение современных рекомендаций по сопроводительной терапии, включающихлекарственный мониторинг, своевременное введение антидота (фолинат кальция), поддержание щелочного уровня pH крови и мочи, объема инфузионной терапии, не всегда оказывается эффективным для профилактики органной токсичности МТX. Актуальными являются исследования по определению спектра токсичности и выявлению возможных факторов прогноза развития МТX‑ассоциированной токсичности.</p><p><bold>   Цель исследования</bold> – анализ спектра непосредственных токсических явлений при терапии неходжкинских лимфом с включением высокодозного МТX, а также определение возможных предикторов выявленной токсичности.</p><p><bold>   Материалы и методы. </bold>С 2020 по 2024 гг. в исследование вошли 100 пациентов детского возраста (до 18 лет) с впервые установленным диагнозом того или иного варианта неходжкинских лимфом, терапия которым проводилась по протоколам противоопухолевого лечения (ALL IC‑bFM 2009, b‑NHL‑bFM 95 либо АККЛ‑НИИ ДЛОГ 2003), включающим высокодозный МТX. Проведены анализ спектра и степени токсических явлений с использованием шкал токсичности Национального института онкологии (США), статистический анализ распределения признаков и степени связи.</p><p><bold>   Результаты. </bold>Гепатотоксичность III–IV степени отмечалась у 49 % пациентов, нефротоксичность III степени – у 1 %. Гематологическая токсичность I–II степени зарегистрирована в 8 % случаев, III–Iv степени – в 92 %. Нейротоксичность развилась у 9 % пациентов: I степени – у 2 %, II степени – у 1 %, III степени – у 3 %, IV степени – у 3 %. Мукозит III–IV степени развился у 30 % пациентов; инфекционные осложнения III–IV степени – у 74 %. Ни в одном случае токсичность, ассоциированная с лечением, не стала причиной нарушений временных сроков введения цитостатических агентов и летальных исходов. У 30 % пациентов отмечалась задержка элиминации МТX &gt;54 ч после начала введения препарата. При анализе влияния дозы МТX на время элиминации и частоту токсических эффектов выявлено, что чем выше дозировка препарата, тем выше частота случаев задержки элиминации МТX и частота токсических явлений.</p><p><bold>   Заключение.</bold> Продемонстрирован спектр МТX‑индуцированной токсичности, представленный гематологической токсичностью, мукозитами, гепато‑, нефро‑, нейротоксичностью и инфекционными осложнениями.
