Abstract

Aim. To assess the registered incidence of multiple myeloma in Moscow, describe its age-, gender- and district-specific distribution, and provide a comprehensive clinical and biological characterization of adults with newly diagnosed multiple myeloma using the Moscow City Cancer Registry data for 2022–2025.

Materials and methods. This retrospective population-based registry study included 1,899 adults with newly diagnosed multiple myeloma recorded in the Moscow City Cancer Registry in 2022–2025. Crude and age-standardized incidence rates were calculated. For each administrative district, the annual number of new cases was normalized to the corresponding adult population size and presented as incidence per 100,000 persons. Temporal trends were assessed using regression models accounting for district and population size. The diagnostic interval and completeness of staging were also analyzed.

Results. The median age of patients was 68 [61; 74] years, and 55.1 % were women. Between 439 and 502 new cases were registered annually. No linear temporal trend was identified in the district-level analysis (p = 0.675). Mean annual crude district-specific rates ranged from 4.06 to 7.78 per 100,000 adults. The median diagnostic interval from the first outpatient visit to morphological verification was 19 [7; 54] days; the diagnosis was verified within ≤ 30 days in 61.2 % of cases. The proportion of patients assigned an mSMART 3.0 category increased from 36 % in 2022 to 70 % in 2025, while the availability of R2-ISS (Second Revision of the International Staging System) increased from 20 to 38 %.

Conclusion. This population-based study provides the first comprehensive description of contemporary epidemiological, clinical and biological features of newly diagnosed multiple myeloma in Moscow using the Moscow City Cancer Registry. Age standardization demonstrated a higher registered incidence in men despite the predominance of women in the cohort. The results obtained show increasing implementation of molecular-genetic risk stratification and support the registry as an instrument for population monitoring and improvement of specialized oncohematological care.
